Add a name or name & number to your item. Please note that this only applies to the tee shirts, Long sleeves & hoodies. If adding to the back of more than one item - you will need to fill this out for each item.
The names will be 2" on the back of the item.
The numbers will be 6" on the back of the item
In the note section please indicate what item it getting the name and or number.
